Getting Started

Set up an appointment with one (or more) of our concierge doctors to learn more about the doctor, the practice and see how the program might work for you.

You can meet with as many doctors as you like at no cost and with no obligation to enroll. During this 20-30 minute appointment you’ll be able to ask the doctor questions about the program, learn about the doctor’s background and credentials and discuss what you’d like to get out of this type of program. We invite you to see just how different our practices are from traditional primary care.

When you’ve decided to enroll, simply schedule your first appointment for your comprehensive physical and bring the completed forms to your first appointment. If you’re not sure when you want to schedule your first appointment, you can choose to mail the enrollment forms to us.
